Office Hours

We are in the office Monday through Friday from 8:00am - 5:00pm..

Our office closes for lunch from 12:00pm - 1:00pm daily.

Appointments

Appointments can be scheduled by calling the office during normal business hours at (352) 291-2400

New patients are asked to arrive 15 minutes prior to your scheduled appointment to complete paperwork. You may download and print out the forms and complete paperwork here. Please bring the forms and your insurance card(s) with you to your appointment.

If you have had a CT scan, PET scan, or chest x-ray, you will need to bring those studies with you to your appointment. Your studies can be obtained from the facility where you had the test performed, usually on a CD disc. Faxed or mailed reports are not sufficient.

As a courtesy to other patients and our staff, please notify the office if you are going to be late for your appointment.

Cancellations

To better serve the needs of all of our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.
